The legendary gems should be at least lvl 25 but the higher the better and if you want you can swap out Bane of the Stricken for Gogok of Swiftness to give a bit more cooldown reduction (I did not do that since I already had Stricken leveled up). You also do not need all of the cooldown reduction on everything possible or to have it at max on any of the items (I was missing it on a few items bc I was too cheap to reroll) but it will help and you want it on most of the gear at least. The gear I used had no rend damage at all (did not want to waste the mats changing stats at mystic if it was not needed) but you will want/need enough dps to kill the mobs fast enough to keep the rampage buff up. You do want as many sockets as possible on your gear, the highest level diamonds in all armor sockets (especially helm), and the highest level emeralds in weapons. You do not need to have perfect gear with perfect stats and I was missing most of these stats when I did it on my paragon 750 seasonal barb. You can also wear the zodiac and cube a coe or band of might (ideally you cube the one with the worst stats and wear the others). You can replace the Convention of Elements with a nice Stone of Jordan as long as it gives you the right damage types, a socket, and its stats are high. You can use physical instead of lightning if you swap rend rune to bloodbath and make sure you go for physical damage on gear. If you are having trouble keeping the wrath of the berserker or call of the ancients up try swapping out rampage for boon of bul kathos.
